Academic conduct report application form
The purpose of the form is to enable students to request an Academic Conduct Report for the purpose of applying for admission to the Victorian Legal Admissions Board.
Academic Transcript request
Use this online form if your course has been conferred and you want to purchase additional copies of your official Academic Transcript.
Application for Supplementary Assessment (Last to Complete) in Final Unit of Study
Use this form if you have obtained a pass result in all units of study for your award except one (criteria applies).
Conceded Pass Application
Use this online form to apply for a conceded pass in a unit or to have a conceded pass removed.
Cover sheet for submission of work for assessment
A completed and signed cover sheet for submission of work for assessment is required for hard copy submissions of work for assessment. For more information see How to submit work.
Examination clash notification form (examinations with same time and same day)
Use this form to indicate details of clashes on your examination timetable and your preferred order of exams.
Special Consideration in assessment (Vocational Education and Swinburne Professional)

This form gives written authority to staff from Swinburne University's to OBTAIN AND RELEASE information relevant to your study requirements / enrolment to individuals or members of the organisations.
Letter of Completion request
Use this online form to request a letter of completion.
Statement of Attainment request (Vocational Education)
Use this online form to apply for a Statement of Attainment. This statement is issued to students who have completed modules towards their accredited course but who have not completed their award.

Enrolment amendment
Application for Advanced Standing - Credit Transfer - Vocational Education (VE)
Credit may be for granted advanced standing if you have completed the same or equivalent unit at another educational institution. Read the information on Advanced Standing then submit the application form and supporting documentation.
Application for Advanced Standing Credits or Higher Education Recognition of Prior Learning (RPL) (Higher education)
Credit may be for granted advanced standing based on previous study you have undertaken at a university, TAFE or equivalent institution. Read the information on Advanced Standing then submit the application form.
Application for incoming cross-institutional study / single unit of study
Use this form if you are not enrolled at Swinburne but wish to undertake a single units of study or are currently enrolled in an undergraduate or postgraduate program at another university wishing to undertake Cross Institutional Study at Swinburne University of Technology
Application for transfer to Commonwealth supported place (Higher Education)
Use this form to request a transfer to a Commonwealth supported place. You have to have completed one year full time study or 100 credit points in your current program and have achieved a credit average.
Application to enrol by proxy (Not applicable for International Student enrolments) (Higher Education)
Give the person who will enrol on your behalf this completed form to bring. Ensure they bring their photo ID and are able to make subject choices for you and are able to pay your fees.

Application to study units at another institution (outgoing cross-institutional study)
Use this form if you are currently enrolled at Swinburne and wish to study at another tertiary institution for credit towards your Swinburne program.
Course transfer request - International students
Current international students wishing to transfer to another Swinburne course must have completed a minimum of one semester of study before requesting a transfer.
Local students can submit a course transfer request by using the online application system.Course transfer request - Local students
Current local students wishing to transfer to another Swinburne course can utilise the online application system. Students need to log in using their existing student ID and password.

This form is for OUA students only who would like to transition into the new version of an accredited Swinburne course.
Domestic students should submit a new course application via eApplication.Enrolment Amendment
Use this online form to change units or a component of your current program, to withdraw from units after the census date, to overload your enrolment or for international students who request to underload from a full time load.

Leave of Absence Medical Impact Statement
Print this form and take it to a Professional Practitioner to complete on your behalf to apply for Leave of Absence due to illness or other special cause.
Pre-training Review
Vocational education (TAFE) student must complete the pre-training review before enrolling into your course.
Prerequisite waiver
Use this form if you wish to enter more advanced areas of study, having already completed the material covered in enrolled units
Submit your Unique Student Identifier
Vocational Education (TAFE) students must submit your Unique Student Identifier (USI) when enrolling into your course. Higher Education students applying for Commonwealth assistance (Commonwealth Supported Place (CSP) and/or HELP loan) must submit your Unique Student Identifier (USI).
Withdrawal from Course Application
Use this online form to notify of your intention to withdraw from your course.
Note: your withdrawal may incur both academic and financial penalties. Read the conditions in this application before submitting.
Fees, Charges and Refund
Application for post-Census remission of fees due to special circumstances
Use this form if Census date has passed, and you need to apply for a remission of your fees due to special circumstances out of your control. If you are an Open Universities Australia (OUA) student and are not on a Commonwealth Supported Place (CSP), you must apply via the OUA website.
Application to be considered as a pre-2008, pre-2009 or pre-2010 Commonwealth supported student
Use this form if you are enrolling into a HECS place at Swinburne and have been previously enrolled in a Higher Education program. Being a pre-2008 students can mean a lower fee.
Application to waive late payment of fees
Use this online form to appeal the late fee incurred as a result of failing to pay fees by due date.
Confirm your Refund Requirements form (Domestic Students)
Use this form if you are a domestic student requesting a refund.
Copy receipt request
Use this online form to request copies of previously issued receipts. These receipts are only provided for payment of tuition fees.
Fee payment extension form (international students only)
Use this online form if you are a continuing student and need to apply for an extension of payment due date. You must apply prior to the due date with accompanying proof of financial hardship. Students in their first year are not eligible.

Skills First program: evidence of student eligibility and student declaration
Use this form to provide evidence of your eligibility for the Skills First program (formerly the Victorian Training Guarantee).
Sponsorship Agreement Form
This form must be completed by the sponsor i.e. employer or other organisation who will be paying fees for a Domestic Student. Both the student and sponsor must agree to the Sponsorship Agreement Terms and Conditions. This form should be completed prior to the student enrolling in a course. International Student Sponsors should use the Sponsorship Registration Form.
Graduation
Late request to attend a Graduation Ceremony
If you have missed the closing date to apply for the next Graduation Ceremony, complete this online form to lodge a late application.
Posthumous / Aegrotat application for award
Use this form to complete a request on behalf of a student who, because of death or severe incapacity, is precluded from completing an award. The student must normally have had no more than the equivalent of one semester of full-time study remaining.
Replacement AHEGS Request
Use this online form to request a replacement Australian Higher Education Graduation Statement (AHEGS) document.
Request for replacement Testamur
Use this form to request a replacement testamur either due to loss or change of name. Payment and supporting documentation is required.
